HealthSpring 2027: Which Plans Pay Less or Nothing
Non-commissionable, partial-commission, and paper-app-only plans for 2027, plus changes to appointment and non-enrollment fees
You find the right HealthSpring plan for a client, submit the application, and learn afterward that it doesn't pay a commission in 2027, pays less than you expected, or can't be submitted on your enrollment platform at all. Checking the list before your appointments saves you from all three.
Starting with applications effective January 1, 2027, HealthSpring is changing compensation on a select list of Medicare Advantage plans and updating how it handles appointment and non-enrollment fees. Most HealthSpring plans are unaffected: of the 203 active 2027 MA plans in HealthSpring's plan listing, 154 pay full commission as usual.
What's Changing
Non-commissionable plans: 26 plans won't pay commissions in 2027 and will be removed from all broker enrollment platforms.
Partial commission plans: 10 smaller plans will pay 75% of the standard fair market value (FMV) first-year commission, including the HealthSpring True Choice (PPO) in Texas.
Full commission, paper app only: 13 plans pay full commission but are removed from broker enrollment platforms, so they must be submitted on paper.
Non-resident appointment fees: HealthSpring will no longer pay non-resident state appointment fees for local brokers after January 1, 2027.
Non-enrollment service fees: Agency non-enrollment service fees will be prorated starting with January 1, 2027 effective dates.
HealthSpring says it's paying 75% of FMV on this limited set of plans to better understand the impact on growth, member retention, and plan performance, and it will monitor results through AEP to decide whether future changes are needed.

Full 2027 Plan Listing
HealthSpring's 2027 MA Plan Listing spreadsheet shows every active 2027 plan with its counties, plan status, and commission status. A second tab lists 2026 plans that are terminating, along with the 2027 plan they crosswalk to, where there is one.
Non-Resident Appointment Fees
HealthSpring says its compliance reviews found that applications taken by local brokers in their non-resident states are more likely to generate complaints. HealthSpring will continue to pay new and renewal resident state appointment fees for properly licensed and certified MA and PDP brokers, but starting January 1, 2027, it will no longer pay new or renewal non-resident state appointment fees for local brokers.
Brokers are responsible for their own non-resident appointment fees incurred after January 1, 2027. Those fees will be applied to the broker's payee balance in Evolve, HealthSpring's contracting and commissions system.
Keep your non-resident appointment active in any state where you receive per member per month (PMPM) commission payments.
To end a non-resident appointment you no longer need, email the Agent Resource Center (ARC) at [email protected].
Non-Enrollment Service Fees
HealthSpring will continue to pay non-enrollment service fees for MA and PDP members at the initial compensation rate when the member meets the "New to HealthSpring" criteria, even if CMS identifies the member as being in a renewal compensation year. Starting with January 1, 2027 effective dates, agency non-enrollment service fees will be prorated based on how many months the member is expected to be enrolled in a HealthSpring plan during the compensation year.
